What: MAHA registration for the upcoming winter season.

Who: Anyone/everyone planning to take part in the Mini Mite, Mite, Squirt/U10, Peewee/U12, or Bantam/U14 programs.

When:

If unable to attend registration prior arrangements  need to be made by contacting Holli Loe 345-5458 or holli.loe@hotmail.com  to avoid a late fee of  $25.00.

New registrants NEED to bring a COPY of their  county birth certificate. 

Where: Sports Institute (Madison East Mall)

Action Needed:

*registration of all player's interested in the 2009-2010 season

*signature on REQUIRED fundraising commitments and pick up calendars

*signature on picture waivers

*opportunity to view and order new MAHA clothing

*opportunity to rent, return, or purchase used equipment

*need a copy of the county issued birth certificate for all NEW members

Registration Fees:

Mini-Mites:  $140.00

Mites:  $170.00

Squirts/U10:  $485.00

Peewees/U12:  $650.00

Bantams:  $725.00

Payment Policy:

Option 1: 100% of registration fees and team fees due at registration

Option 2: Pay in 2installments - 50% due at registration, 50% due at your individual team meeting after placements.

Members will be able to make payments via credit card at registration.

*Financial assistance will be made available at registration through Southern MN Children's Project*

Miscellaneous Notes:

**Information on preseason meetings, fall hockey, and tryouts will be available.

We offer reasonable equipment rental and scholarships.
